- Have an JHS approved manuscript proposal or ancillary study application
- Have completed and executed JHS Data Material Distributed Agreement (DMDA)

An executed DMDA implies that you completed the document, signed by an official
at you institution, and signed by JHS partnering institution and NHLBI
- Have no outstanding violations of DMDA stipulations, Presentations and Publications
Policies and Guidlines, or Ancillary Studies Policies and Guidelines
You may proceed with your request only if all three of the above requirements have
been met. If you have not met the above three requirements you may
click on the "Decline Request" button.
